What Happened
A number of hospitals in Democrat-leaning states have curtailed or ended gender-affirming care programs for minors — including puberty blockers and hormone therapy — amid a wave of executive orders and federal funding threats from the Trump administration. Families who relied on these programs are now seeking care elsewhere or going without, as providers weigh legal and financial risk against patient need.
Historical Context
This is a direct consequence of an executive order signed in January 2025 directing federal agencies to end such care at federally funded institutions. Dozens of major children's hospitals — including those in states with no state-level bans — have since paused or ended services. Legal challenges are ongoing: the ACLU and other groups have filed suits, and lower courts have issued mixed rulings. Historically, access to contested medical treatments for minors has been regulated unevenly during legal transitions — similar patterns occurred with abortion access between Roe (1973) and Dobbs (2022), where care disappeared in some states before courts settled the question. The number of minors receiving gender-affirming hormones or surgery in the U.S. was estimated at roughly 14,000–18,000 annually before these restrictions began.
